Hello. I have an Insignia 39" TV, about two years old, Model #NS-39D22ONA16.
I have never had it connected to a cable box, just an over-the-air indoor TV antenna.
It has suddenly stopped receiving OTA signals... reception up until now has been excellent. I had not moved or tampered with the TV at all. Channel re-scans produced no results. I even hooked up a different indoor antenna, but I still got no results upon re-scanning.
I also powered the TV and turned it back on, but still nothing.
Any suggestions? Maybe reset it to factory defaults?

I would recommend power-cycling your TV by turning it off and unplugging it for several minutes. This should serve to reset the TV's hardware.
Additionally, as you suggested, you may try a reset through the TV's settings. You can find instructions for completing this process in the TV User Guide.
